The Difference Between Home Health and Hospice
Home health and hospice are both involved in caring for patients in their own homes. Both services are dedicated to respecting patient rights and decisions and involving family members in the patient’s care. Both services are also committed to making their patients comfortable and improving their quality of life. Both home health and hospice can be beneficial to patients and their families at different stages of a patient’s illness progression. Home health provides the majority of its services at the beginning of care and tapers off as the patient becomes more independent. Hospice provides the majority of its services at the end of care, when the needs of the patient and family increase.
Entera Home Health and Hospice is proud to offer both services. Our team assists patients and their families throughout their continuum of care from diagnosis and throughout disease progression. We are committed to providing the highest quality of services to patients based on their unique needs, and we strive to provide patients with what they need at the time that they need it.
Entera Home Health provides an array of services to help patients recover from illness or injury, restore function, and/or learn to live with chronic disease. Under the care of concerned professionals, patients are given the support they need to recover or stabilize in their own homes.
If a time comes when home health is no longer able to provide optimal care due to progressive illness, Entera Home Health can ensure a continuation of care excellence by transitioning to Entera Hospice.
Our specialized hospice team is available to help with the challenges associated with end of life care. Our team can work with patients and their families to develop a plan of care particularly suited to meet your needs. Entera Hospice helps relieve physical symptoms and other burdens associated with illness so that patients and families can focus on living life to the fullest.
Entera Home Health and Hospice’s primary goal is to ensure the best quality of care for the best quality of life for you and your family. A transitions coordinator can help you identify the level of care most appropriate to meet your needs.
